The Molina Medicare Medication Therapy Management Pharmacist provides a clinical pharmacy outreach service to educate patients and their health care team in order to optimize medication related heal! thcare outcomes, ensure patient safety, recommend cost-effective medication strategies, coordinate care with the member's interdisciplinary care team and comply with CMS guidelines.

Essential Functions

CMS:

• Comply with all CMS guidelines for the Medication Therapy Management Program (MTMP)
• Fulfill all reporting requirements required by CMS for the MTMP
• Fulfill all process and outcome measurements as defined in pharmacy department policy number PD-3

Medication Therapy Management:

• Provide a comprehensive medication review for eligible members annually
• Analyze and review medication lists for potential patient safety problems, including potential drug interactions, suboptimal medication regimens, etc.
• Make outbound calls to members and speak with them about their medications
• Provide education to members and their health care providers about the members' medications
• Assist in coordination of care for pharmacy-related i! ssues between members, providers and other members of the member's interdisciplinary care team, including referrals to state plan health care services
• Communicate identified interventions to members' providers
• Document interactions with members and providers
• Send a reconciled personal medication list to members once comprehensive review is completed
• Follow-up with members and providers to ensure changes are being made to meet treatment goals and to determine which recommended changes to drug therapy were implemented
• Demonstrate proficiency on all technological computer programs and platforms used in the MTMP
• Provide drug information to Molina Medicare enrollees as requested

Knowledge/Skills/Abilities

• Current working knowledge of best practices in MTMP-related disease states
• Pharmacotherapy expertise for MTMP-related conditions and medications
• Excellent verbal and written communic! ation skills, friendly voice over the telephone
• Ability to abide by Molina's policies
• Maintain regular attendance based on agreed-upon schedule
• Maintain confidentiality and comply with Health Insurance Portability and Accountability Act (HIPAA)
• Ability to establish and maintain positive and effective work relationships with coworkers, clients, members, providers and customers

Required Education:

• Doctor of Pharmacy from an accredited pharmacy school
• Continuing education required to maintain an active pharmacist license

Required Experience:

• Clinical skills related to medication therapy management
• Training in Medicare Part D

Required Licensure/Certification:

• Maintain an active state pharmacist license

Preferred Education:

• Ambulatory Care Residency

Preferred Experience:

• Me! dication therapy management

Preferred Licensure/Certifica! tion:

• Board Certified Ambulatory Care Pharmacist
• American Pharmacists Association Medication Therapy Management Certificate Training Program certificate or equivalent
