This position will serve as a Clinical Pharmacist within the Medicaid Pharmacy Department and will be supporting! the coverage determination processes and MTM services. .
Review requests from members and physicians for Coverage Determinations.
Assists with the analysis of drug utilization data to identify trends and develop strategies.
Work with the State of Illinois to develop clinical guidelines and assessments for use in the Prior Authorization Process.
Develop and present educational materials to members and physicians.
Handles phone inquiries from both internal (Markets, Specialists, Medical Directors) and external (Pharmacists, Physicians) customers.
Work with an interdisciplinary team to develop a patient centered care transition programs to reduce hospital admissions/re-admissions.
Perform telephonic MTM reviews and clinical assessments directly with members.

Requirements
Licensed Pharmacist in the State of Illinois
Degree in pharmacy from an accredited school of pharmacy, advanced ! clinical degree (PharmD or clinically-oriented Master’s Degr! ee preferred)
PBM/Managed Care experience in clinical administration of prescription benefits
Medication Therapy Management (MTM) expertise
