Job Description

Job Title: Director
Department: Pharmacy
Reports To: Chief Nursing Officer
FLSA Status: Exempt

SU! MMARY
Compounds and dispenses prescribed medications, drugs, and other pharmaceuticals for patient care by performing the following duties. Directs, coordinates and supervises all aspects of the Hospital Pharmacy. Directs and supervises all the personnel working within the pharmacy.

ESSENTIAL DUTIES AND RESPONSIBILITIES

  • Reviews prescriptions issued by Physician, or other authorized prescribers to assures
accuracy, proper dosing, and formulary compliance.

  • Compounds medications, using standard formulas and processes such as weighing, measuring,
and mixing ingredients.

  • Directs pharmacy technicians engaged in mixing, packaging, and labeling, and preparing of
pharmaceuticals.

  • Answers questions and provides information to patients, employees, medical staff, and nursing staff
on drug interactions, side effects, dosage and storage of pharmaceuticals.

  • Maintains established policies and procedures as! noted in pharmacy manual, Performance
Improvement requirements and all state and federal laws.

  • Responsible for all pharmacy related computer activities.
  • Verifies all computer entries made by technicians.
  • Inputs and maintains pharmacist intervention information in computer.
  • Reviews and updates lab information relating to medications in the pharmacy computer.
  • Participates in pharmaceutical care including discharge planning, lab monitoring, intervention
counseling, etc. as per pharmacy policies.

  • Ensures pharmacy is in compliance with all applicable rules and regulations.
  • Maintains all IV preparation rules and regulations.
  • Responsible for all drug ordering and storage in pharmacy and throughout the Hospital.
  • Responsible for maintaining the professional appearance of the pharmacy, except for routine
housekeeping duties which are performed by housek! eeping staff.

  • Responsible for maintaining accurate computer information in computer including pricing and clinical.
  • Maintain pharmacy operation within budget.
  • Ensures proper staffing levels to meet hours of operation for hospital.
  • Serves on Hospital committees as designated. Hospital staff person responsible for Hospital P and T
committee, scheduling , agenda development and meeting minutes. Co-chair with staff appointed
MD. Responsible for scheduling and conducting Medication Error Committee.

  • Responsible for annual policy review as well as development of new policies.
  • Participates in Performance Improvement Activities.
  • Prepares and submits budget, records, reports and statistical data to CNO and Hospital
Administrator.

  • Directs Pharmacy initiatives to ensure readiness for IDPH surveys and monitors compliance with
conditions of participation! for Critical Access Hospitals.

  • Participates in all s! afety programs which may include assignment to an emergency response team.
  • Performs other duties as assigned by Supervisor.

SUPERVISORY RESPONSIBILITIES
Supervises technicians and staff pharmacist, responsible for maintaining licensed pharmacist coverage when not on duty.

QUALIFICATIONS

EDUCATION and/or EXPERIENCE
Completion of pharmacy program from an accred! ited school.

LANGUAGE SKILLS
Ability to read, analyze, and interpret common pharmacy and medical journals. Ability to respond to common inquiries or complaints from customers, or regulatory agencies.

MATHEMATICAL SKILLS
Ability to apply routine pharmacy mathematical skills.

REASONING ABILITY
Ability to define problems, collect data, establish facts, and draw valid conclusions.

CERTIFICATES, LICENSES, REGISTRATIONS
Current IL pharmacist license. Meet continuing education requirements as per Department of Professional Regulation.

Qualifications & Requirements:
  • Pharmacy Degree (BS or PharmD)
  • Current IL license in good standing
  • 3-5 years experience as a Pharmacist
  • Previous Hospital experience preferred
  • Previous Management experience preferred
