Edward Hines, Jr. VA Hospital, located 12 miles west of downtown Chicago on a 147-acre campus, offers primary, extended and specialty care and serves as a tertiary care referral center for VISN 12. Specialized clinical programs include Blind Rehabilitation, Spinal Cord Injury, Neurosurgery, Radiation Therapy and Cardiovascular Surgery.
Hines VAH currently operates 471 beds and six community based outpatient clinics in Kankakee, Elgin, Oak Lawn, Aurora, LaSalle, and Joliet. Over 600,000 patient visits occurred in fiscal year 2010 providing care to over 54,000 veterans, primarily from Cook, DuPage and Will counties.
Hines VAH is affiliated with 70 colleges and universities and 40 associated health professions. The main medical school affiliation is the Stritch School of Medicine at Loyola University of Chicago. There are over 1,300 students educated annually at Hines.

DUTIES:
Work Schedule: Monday - Friday 7:30am - 4:00pm
This RN position is located at the La Salle CBOC.
Registered nurses assigned to a Patient Aliened Care Team (PACT) Model are members of a teamlet assigned to a panel of patients. The RN functions as a member of a team consisting of the patient, the patientÂ's primary care provider, the teamlet LPN, the teamlet clerk and other professionals including Clinical Pharmacists, Dietitians, Psychologists, and others.
Optimizes patient care delivery and patient outcomes by providing Case Management; Care Coordination; and Telephone Management.
Provides care to patients with acute problems, patients undergoing transitions of care and patients with chronic conditions who would benefit in developing and performing self management skills for chronic conditions especially diabetes, hypertension, CHF, hyperlipidemia, pain, depression.
Use a registry to identify high risk patients who would benefit from case management.
Utilizes motivational interviewing techniques to facilitate patient development of patient driven goals for health optimization.
Performs independently in collaboration with the provider to carry out the plan of care for the veteran. He/she appropriately delegates to other team members but is responsible for providing case management and care coordination activities, clinical triage and making more complex clinical decisions.
Uses face to face visits, telephone visits, secure messaging, and possible inpatient visits to patients assigned to the teamlet.
Provides backup for other teamlet RN staff. The RN tracks patient outcomes to determine if interventions are effective.
QUALIFICATIONS REQUIRED:
QUALIFICATIONS:
- Must be a citizen of the United States.
- Graduate of a school of professional nursing approved by the appropriate State-accrediting agency and accredited by one of the following accrediting bodies at the time the program was completed by the applicant: The National League for Nursing Accrediting Commission (NLNAC) or The Commission on Collegiate Nursing Education (CCNE)
- Current, full, active, and unrestricted registration as a graduate professional nurse in a State, Territory or Commonwealth (i.e., Puerto Rico) of the United States, or the District of Columbia. In cases of graduates of foreign schools of professional nursing, possession of current, full, active and unrestricted registration will meet the requirement of graduation from an approved school of professional nursing.

- Previous Outpatient nursing experience
EDUCATION/EXPERIENCE REQUIREMENTS:
Nurse I Level I - An Associate Degree (ADN) or Diploma in Nursing, no experience.
Nurse I Level II - An ADN or Diploma in Nursing and approximately 1 year of experience, or an ADN or Diploma in Nursing and a bachelor's degree in a related field and no experience; or a Bachelors of Science in Nursing (BSN) and no experience.
Nurse I Level III - An ADN or Diploma in Nursing and approximately 2-3 years of experience, or an ADN or Diploma in Nursing and a bachelors degree in a related field and approximately 1-2 years of experience; or a BSN with approximately 1-2 years of experience, or a Master's degree in nursing (MSN) or related field with a BSN and no experience.
Nurse II - A BSN with approximately 2-3 years of experience, or ADN or Diploma in Nursing and a bachelors degree in a related field and approximately 2-3 years experience or a Master's degree in nursing or related field with a BSN and approximately 1-2 years experience, or a Doctoral degree in nursing or meets basic requirements for appointment and has doctoral degree in a related field with no experience.
Nurse III - Master's degree in nursing or related field with BSN and approximately 2-3 years experience or Doctoral degree in nursing or related field and approximately 2-3 years experience.
