This position provides pharmaceutical care services to all patients of the hospital including, but not limited to, oversight, monitoring, preparation, dispensing and proper documentation of patient medication therapy as well as providing drug information a nd oversight over information and dispensing systems. Pharmacists will work with the pharmacy team to ensure compliance with Federal, State and local laws and reguations, as well as proper storage and documentation of pharmaceuticals. This position supervises and directs support personnel. In the absence of a pharmacy manager, this position may be required to assume some of the essential responsibilities and duties of the manager. II. Position Responsibilities: No. Percent of Time Essential Duty Job Duties 1 55% Yes Process medication orders for age-appropriate and disease-appropriate medication therapy. a) Screen medication orders for the correct dose, the absence of drug interactions, contraindications or allergies, and for the appropriateness of therapeutic and age specific parameters. b) Verify orders prescribed by physicians or other licensed independent practioners entered through the Epic system after screening for above has been completed. c) Check and/or fill medica tion orders to provide for the therapy of patients of all ages. d) Resolve questionable orders to provide safe, effective patient drug therapy. e) Respond to changing priorities and emergent situations to provide for critical patient care needs. f) Identify areas of potential expense reduction and cost savings to optimize departmental budgetary performance g) Perform technical tasks that are new or complex in the absence of pharmacy technicians or to provide continuous pharmacy service. h) Monitor and trouble shoot problems associated with the dispensing technologies and information systems. i) Participates in pharmacy department driven protocols, such as IV to PO conversion and renal dosing. j) Documents medication interventions in the designated on-line system. k) Participates in the quality improvement/qualtiy control activities of the department. l) Interpret information in the electronic medical record in order to make appropriate recommendations regarding medication th erapy. m) Provide meal recommendations regarding medication therapy. m) Provide meal coverage for satellite areas, such as OR, ASC and Peds. n) Staff in satellite areas such as ASC, OR and Peds 2 30% Yes Supervise the work of Pharmacy Technicians to ensure the delivery of high quality pharmaceutical services. a) Check the work of pharmacy technicians in the IV admixture, TPN admixture and unit dose dispensing areas to ensure accuracy and age appropriateness of the preparation. b) Monitor work flow through the IV admixture, TPN admixture and unit-dose dispensing areas to ensure the timely provision of pharmaceuticals. c) Take responsibility for changing technician work assignments and tasks to accomodate fluctutating staffing patterns and work load demands, especially in the absence of managerial personnel. 3 10% Yes Provide medication information to other health care team members to assist them in their patient care activities. a) Provide medication information to other heal th care professionals that is accurate, adequate and timely to optimize the patient's drug therapy . b) Explain procedures and optimal pathways to achieve understanding and patient care goals. c) Negotiate solutions to pharmacy service conflicts that result in achievement of patient care priorities. 4 5% Yes Act as an interface between pharmacy and its customers to coordinate the delivery of Pharmacy services. a) Assist Pharmacy customers to satisfy their medication and information needs. b) Counsel patients concerning their medications so they will understand their drug therapy and maximize its effectiveness. c) Participate in continuing education to maintain up to date skills and knowledge. d) Provide information on and practice according to controlled substances regulations and pharmacy practice to ensure compliance with the law.
