Primary Responsibilities:
Provide a single point of contact for a CCG / group of GP practices for medicines optimisation
To be highly visible, accessible and responsive, building strong relationships, especially with GPs, and becoming a familiar and trusted resource
Deliver strategic plans and input technical pharmaceutical expertise as well as ensuring implementation of MMO QIPP plans, practice based support and education and training
Work in GP practices, working closely with GPs, other practice staff and members of the primary healthcare team in supporting the development and application of high quality, safe, cost effective, rational and evidence based prescribing
To conduct appropriate medication review for identified patients, communicating directly with patients as required
Provide pharmaceutical advice on a day to day basis in all aspects of prescribing and medicines management to GPs, nurses, other practice staff, and patients as appropriate. This will include answering queries / acting as an information resource
Provide advice to practices on matters that influence prescribing e.g. managing pharmaceutical company representation, reviewing promotional literature, providing evidence-based and impartial assessments of claims made in support of products
Carry out prescribing based audit in line with Medicines Optimisation Team priorities
Advise practice staff on audits relating to prescribing where appropriate. The data collection and reviewing of patient records will be in accordance with the latest information governance, Data protection and Caldicott guidelines. The post holder will work in Lincolnshire CCG GP practices through local, embedded arrangements for providing specialist support on their primary care prescribing
Present information in a non-confrontational manner in one to one situations, small groups and large audience, which on occasion be in adversarial conditions and articulating outcomes
Liaise with local community pharmacists, community nurses, community matrons, care home staff, hospital pharmacists and other secondary care providers as necessary to ensure accurate exchange of information / clarification of queries regarding medicines. This may also include speaking to consultants/ward staff concerning discharge medication
Review practice prescribing patterns and discuss recommendations with prescribers
Negotiate change in prescribing practice with GPs and non-medical prescribers
Assist the GPs and practice team in implementing national and local policies and guidance relating to prescribing, and in development of practice based policies, procedures and guidelines relating to prescribing
Support the implementation of the local joint prescribing formulary
Analyse, interpret and monitor practice prescribing and cost data (EPACT data) (with data from the PPD direct or from the Optum MMO team as appropriate) and make appropriate recommendations, including in relation to areas of prescribing where the practice is an outlier within the CCG
Carry out medication reviews, discuss options and make recommendations to prescribers as appropriate. This may include patients in nursing and residential homes
Review patient medication from GP clinical records, analysing both drug and patient information and advising GPs on suitable drugs / action and drug monitoring where appropriate
Analyse / audit the repeat prescribing systems within designated Practices and develop, as necessary, improvements in the management of repeat prescribing
Review repeat prescriptions for rational, safe, and cost effective prescribing
Discuss with the doctor and recommend changes as appropriate
Assist in managing the introduction of new drugs and drugs that are discontinued
Assist the Medicines Optimisation Team in the identification and monitoring of prescribing- related indicators and targets to demonstrate adherence to formularies, prescribing trends etc.
Assist the practice in ensuring that systems and procedures relating to prescribing and the management of medicines comply with legal and best practice standards, making recommendation where appropriate
Advise on storage of pharmaceuticals within the surgery
Support the implementation in practices of national medicines management initiatives such as the Repeat Dispensing Scheme, Medicines Use Reviews and New Medicines Service
To identify initiatives and support the practices in attempts to minimise waste of medicines
Undertake any other reasonable duties as may be assigned from time to time by the Head of Medicines Optimisation
Support the delivery of relevant training and development relating to medicines use, including organising and delivery of training and education

Required Qualifications:
MPharm in pharmacy or equivalent
Registered with the General Pharmaceutical Council
Evidence of Continuous Professional Development (CPD)
Strong post registration relevant experience
Extensive experience in clinical pharmacy and drug utilisation review
Experience of inter-disciplinary working
A record of achievement in delivering objectives within required timescales
Full driving licence and access to transport
Willingness to travel within Lincolnshire, and neighbouring areas if required
Experience of working in GP practices and primary healthcare teams
In depth therapeutic and clinical knowledge and understanding of the principles of evidence based healthcare
A sound understanding of the nature of primary care prescribing, concepts of rational prescribing and strategies for the improvement of medicines optimisation overall
Understand the concepts of evidence based medicine, needs assessment, effectiveness and prioritisation
Understanding of commissioning processes
Understanding principles of clinical governance and how these apply to the broader agenda
Able to demonstrate excellent organisational skills, and an ability to prioritise and work to deadlines
Able to communicate clearly and concisely
Excellent verbal and written skills
Highly numerate
Competent in use of standard IT and relevant software
Frequent requirement for concentration when checking documents
Able to demonstrate commitment to team working

Preferred Qualifications:
Experience in developing and delivering education and training
Accredited post-graduate clinical pharmacy diploma or MSc or equivalent postgraduate qualification
Experience of working in both primary and secondary care settings
Knowledge of medicines optimisation issues relating to care homes
