DUTIES:
The successful candidate will function as a clinical pharmacy specialist with a prescriptive scope of practice to initiate, change or discontinue OTC or prescribed medication for the patient involved in the clinic. The activities are patient and practitioner related, and requires an advanced degree of skills in patient monitoring and drug therapy management. Clinical duties involve improving the provision of clinical pharmacy services for facilities within VISN 12 as related to post-transplant management and primary care related conditions. The candidate will function as a point of contact for post transplant medication management and may also use virtual care modalities which include clinical video telehealth, secured messaging, and telephone care. Involvement w! ith VISN/national initiatives and workgroups is also undertaken to improve pharmacy services overall approach to patient-centered care. The incumbent will be involved in policy development related to transplant initiatives, formulary conversions, medication use evaluations, medication reconciliation, quality assurance reviews, patient counseling, healthcare provider education and additional duties as assigned.

Work Schedule:
Monday - Friday, 7:30 AM - 4:00 PM, Some travel is required.

QUALIFICATIONS REQUIRED:
To qualify for this position,

BASIC REQUIREMENTS:
U.S. Citizenship.

You must be proficient in spoken and written English.

EDUCATION:
Must be a Graduate of an Accreditation Council for Pharm! acy Education (ACPE) accredited College or School of Pharmacy with a ba! ccalaureate degree in pharmacy (BS Pharmacy) and/or a Doctor of Pharmacy (Pharm D.) degree.

OR

Must be a Graduate of a foreign pharmacy degree programs that meet the educational requirement and am able to provide proof of achieving the Foreign Pharmacy Graduate Examination Commission (FPGEC) Certification, which includes passing the Foreign Pharmacy Graduate Equivalency Examination (FPGEE) and the Test of English as a Foreign Language Internet-Based Test (TOEFL iBT).

LICENSURE:
You must possess a current, full, active, and unrestricted license to practice pharmacy in a State, Territory or Commonwealth (i.e., Puerto Rico) of the United States, or in the District of Columbia.

SPECIALIZED EXPERIENCE:
You must have the equivalent of 1 year of current creditable experience at the next lower level (GS-12) or equivalent, which is directly related to the position to be filled. You must possess one year of experience comparab! le to the GS-12 Level AND meet the KSAs required at the GS-12 and GS-13 Level as described in the Qualifications/Evaluations section of this announcement.

OR

You must have completed 1 year Post Pharm D. ASHP accredited Residency.

KNOWLEDGE, SKILLS, AND ABILITIES:
In addition can demonstrate the following KSAs:
KSA 1: Ability to communicate orally and in writing to persuade and influence clinical and management decisions.

KSA 2: Expert understanding of regulatory and quality standards for their program area.

KSA 3: Ability to solve problems, coordinate and organize responsibilities to maximize outcomes in their program area or area of clinical expertise.

In addition to the basic requirements, candidates must meet one of the following:

Candidates with at least a Master's Degree preferred

·

1 year of experien! ce equivalent to the next lower grade level (GS-12), or

·
Completion of a Resident or Fellow training program in a specialized area of clinical pharmacy practice may be substituted on a year-for-year basis.

Preferred Qualifications:
ASHP-accredited, PGY1 residency completion. Board Certified Pharmacotherapy Specialist or Board Certified Ambulatory Care Pharmacist is desired.

Prior VA experience is preferred.

You will be rated on the following Job Elements as part of the assessment for this position:

Experience

as a clinical pharmacy specialist working independently under a scope of practice.
