Further informations about this job opportunity kindly see the descriptions. Seeking IL Licensed Clinical Pharmacist to work in the IV Pharmacy on a part-time basis (16-24 hours per week)
1. Process routine daily pharmacy orders per SOP's, Federal and ! State requirements.
2. Communicate and coordinate work flow (New Rx, Refills, Change of Orders) with PCC's
3. Coordinate with Physicians, Nurses, Home Health Agencies and patients about orders, change of orders, labs, lab draws, lab results to provide continuity of care.
4. Complete all tasks ad orders via Fastrack, Rx30 IV Census
5. Check and perform verification of IV compounding/production per SOP's
6. Complete Plan of Care and clinical assessments for all patients per The Joint Commission and SOP's requirements.
7. On-call and other related duties as assigned.
Must possess IL Pharmacy License and have a minimum of 3-5 years of experience.
IV Home Care, Hospital, Ambulatory or Retail experience preferred.
