The Antibiotic Stewardship Pharmacist (ASP) is responsible for the development, implementation, and documentation of the antimicrobial stew! ardship program for the network. This position, in collaboration with the Infectious Disease Division of the Dept of Medicine, the Clinical Staff Pharmacists, and the individual Directors of Pharmacy at all locations, will improve antimicrobial drug use and therapeutic outcomes, reduce antimicrobial resistance, and reduce drug costs associated with prevention and treatment of infectious diseases.
REPORTS TO:
This position reports to the VP of Patient Safety and Quality.
MINIMUM QUALIFICATIONS:
Education, Experience, and Professional Licensure
Bachelors degree from an ACPE accredited College of Pharmacy and/or Doctor of Pharmacy degree from an ACPE accredited College of Pharmacy
Formal residency training (PGY1, PGY2-Infectious Disease, or I.D. Fellowship) or equivalent infectious disease experience preferred.
Board Certified Pharmacotherapy Specialist preferred (BCPS AQ-ID or BCPS) or pursued once in the posi! tion.
Minimum 3-5 years experience as a director of p! harmacy, clinical manager or advanced clinical pharmacist in a hospital setting is required.
Minimum of 12 months experience in infectious disease setting is preferred.
Knowledge, Skills, and Abilities.
Must possess excellent interpersonal skills, verbal and written communication skills as well as experience with clinical program development and/or clinical management skills.
Works effectively and productively with shared responsibilities. Supports teamwork in the department and hospital by including participants in the communication of projects and/or activities.
Listens effectively and communicates ideas and opinions clearly.

KEY RESPONSIBILITIES :
In conjunction with the Anti! biotic Subcommittee of the Pharmacy and Therapeutics Committee, the ASP is responsible for the oversight of all antimicrobial drug use as specified below (but not limited to):
Improve Antimicrobial Drug Use and Therapeutic Outcomes
Ensure the use of best practices with regards to pharmacokinetic and pharmacodynamic utilization/dosing of antimicrobials.
Promote the conversion of intravenous to oral antibiotics when appropriate.
Assist in the development of infectious disease related protocols and orders.
Reduce Antimicrobial Resistence
Appropriate prophylaxis, empiric, and therapeutic treatments of patients.
De-escalation of antimicrobial agents to narrow spectrum, pathogen based treatment.
Increase Patient Safety and Reduce Drug Costs
Review formulary and determine which agents to include based upon efficacy, safety, and cost.
Facilitate safe medication practices to reduce error and adverse drug reactions.
Continuously review! drugs of choice and monitor utilization.
Document improvement in p! atient well being and consumer satisfaction by shortening length of stay, preventing secondary infections, and ensuring positive patient outcomes.
