The Registered Professional Nurse in the Community Living Center (CLC) is qualified by education and experience to provide care for assigned residents and supervision of subordinate di! rect care staff in planning, designing, integrating, implement! ing, modifying, and evaluating the effectiveness of interdisciplinary care plan interventions and treatments and/or services to improve the quality of resident care. He/she possesses intimate knowledge of Long Term Care federal regulations, VHA guidelines, medical review criteria, Minimum Data Set, clinical documentation requirements, current standards of care, and compliance guidelines.
On October 1st, 2010, the North Chicago VA Medical Center integrated with the Naval Health Clinic, Great Lakes to form the Captain James A Lovell Federal Health Care Center (FHCC). The facility is located just 13 miles south of the Illinois/Wisconsin border approximately equidistant between Chicago and Milwaukee. Offering a choice of three international airports, the excitement of the 'Big City' with the beauty of the rural countryside, there are countless activities to engage a wide variety of personal hobbies and interests. The facility itself has opened a new parking structure a! nd a new outpatient clinical addition.

English Language Proficiency required. DUTIES:
NOTE: ONE full-time position; Days (0715-1545) with rotation. Rotation means that the day shift will be your primary work schedule. However, in the event of unexpected circumstances, you may be required to work other shifts to meet the mission of the unit and organization.
The CLC RN executes position responsibilities that demonstrate leadership, experience, and creative approaches to management of the long term care resident. The RN functional role includes organizing and coordinating the clinical components in response to individual healthcare needs along the illness and care contin! uum while a resident within the Community Living Center. The goals are ! to center services around the resident, to foster resident independence to their highest functional capacity, and maximize efficient and cost-effective use of health care resources. The RN utilizes all avenues of resources available to provide safe and efficient timely coordination of quality health care and interdisciplinary services.
Specific duties for the RN, Community Living Center inclulde the following:
a. The function of the Registered Nurse is to provide, direct, and supervise the delivery of quality nursing care to a culturally diverse veteran population.
b. The RN possesses the knowledge and skills necessary to effectively apply all aspects of the nursing process within the collaborative, interdisciplinary practice setting of the Community Living Center. This broad based clinical knowledge enables the RN to effectively implement health promotion and prevention interventions, manage acute and chronic illness states, assist veterans in th! e attainment of optimal levels of functioning through rehabilitation, and provide palliative care for the resident and their significant others.
c. Supports the Veterans admitted to the Community Living Center through assessment and evaluation and ensure appropriate follow through with the plan of care as required.
d. Coordinate resident appointments and procedural instructions, follow-up and report test results to the appropriate providers.
e. Effectively collaborates with all levels of the health care team. This includes, but is not limited to clinics, inpatient departments, physicians, nurses, other health care providers such as Pharmacists, Social Workers, Psychologists, Physical Therapists, Dieticians, Recreational Therapy, and other outpatient departments, ancillary services, community resources, etc.
f. Ensures instruction to the resident and the family based on identified learning needs. Assesses the family's knowledge base, healt! h status expectations, and the potential for the family involvement in ! resident care in the Community Living Center. Assists with the development of activities and methods to ensure information is articulated and disseminated to staff.
g. Provides tracking and trending of clinical outcomes and the Minimum Data Set.
h. Clinically supervises and provides guidance to direct care staff while on duty.

Citizen of the United States. Graduate of a school of professional nursing approved by the appropriate State-accrediting agency and accredited by one of the following accrediting bodies at the time the program was completed by the applicant: The National League for Nursing Accrediting Commission (NLNAC) or The Commission on Collegiate Nursing Education (CCNE). Current, full, active, and unrestricted registration as a graduate professional nurse in a State, Territory or Commonwealth (i.e., Puerto! Rico) of the United States, or the District of Columbia . In cases of graduates of foreign schools of professional nursing, possession of current, full, active and unrestricted registration will meet the requirement of graduation from an approved school of professional nursing. Must be proficient in spoken and written English .
Additional Requirements :
( 1) At least ONE (1) year of increasingly complex RN nursing experience.
(2) Current certification in BLS/CPR.
Preferred Requirements:
(1) A Bachelor of Science in Nursing OR a Bachelor's Degree in a health-care related field.
(2) At least SIX (6) months of RN experience caring for chronic/long term care medical/surgical and/or psychiatric patients.
(2) Current certification in Gerontological Nursing by the American Nurses Credentialing Center (ANCC).
Applicant's education and length of nursing practice (experience) will be considered by a Nurse Pr! ofessional Standards Board in determining the grade and salary of the a! pplicant selected. The recommended salary may be at any point in the range listed for this vacancy. Physical/Functional Requirements: The individual must have the physical ability to perform job-related duties which require lifting, standing, bending, transferring, stooping, stretching, walking, pushing, and/or pulling. A health examination must be successfully completed prior to this assignment and periodically thereafter as required by the Employee Health Clinic policy. There will be intermittent exposure to blood and body fluids.
