Clinical Licensure Required : Pharmacist
Job Type: Full Time Position Summary:
Responsible for the accurate delivery of pharmaceutical care services ! including drug therapy monitoring, patient education, drug information and chronic disease state management for our clinical programs. Works as part of interdisciplinary team including registered nurses, clinical pharmacists, and clinical assistants. Required Qualifications:
Pharmacy degree from an accredited college of pharmacy. Active licensure as a practicing pharmacist in Illinois. Familiar with JCAHO standards, State and Federal regulations. Excellent interpersonal skills. Excellent written and verbal communication skills. Ability to work effectively with minimal direction and as part of interdisciplinary team. Ability to provide support in all aspects of pharmacy processes. Extensive knowledge of disease state management, drug information (drug actions, incompatibilities, interactions, dosage ranges, side effects, and proper storage), and legal requirements. Ability to provide effective drug information, counseling, therapeutic interventions, and disease state man! agement. Preferred Qualifications:
Infusion experi! ence a plus. Education:
Degree in pharmacy from an accredited School of Pharmacy and must hold a current license as a Registered Pharmacist in the State of Illinois. Business Overview:
