Clinical Pharmacy Specialists are registered clinical pharmacists who assume primary responsibility and accountability for the provision of safe, effecti! ve, and prompt medication therapy. Clinical Pharmacy Specialists ensure medication therapy is appropriate to achieve outcomes related to the cure, diagnosis or prevention of disease, elimination or reduction of patient’s symptoms, or arresting or slowing of a disease process, and improve a patient’s quality of life in collaboration with a multi-disciplinary team. Clinical Pharmacy Specialists serve as a departmental resource person and liaison to physicians, pharmacists, nurses, and other hospital personnel. Clinical Pharmacy Specialists serve as therapeutic leaders for the appropriate use of medications according to professional and regulatory standards and lead initiatives to detect, mitigate, and prevent medication adverse events. Clinical Pharmacy Specialists will work with the Director of Pharmacy to enhance pharmaceutical services and are responsible for identifying and implementing new clinical services provided by the Department of Pharmaceutical Services.
Clinical Pharmacy Specialists are specifically aligned wit! h key therapeutic areas and patient care service lines core to the University of Chicago Medical Center. Through these alignments the Clinical Pharmacy Specialists provide clinical and distributive pharmaceutical care services for the patients within their assigned areas including assessment of individual patient needs, incorporation of age and disease specific characteristics into drug therapy and patient education, provision of individualized care relevant to patient needs, and delivery and monitoring of interventions as appropriate. Clinical Pharmacy Specialists work to establish strategic relationships with University leaders/stakeholders pivotal to the advancement of assigned patient care service lines and assume responsibility for the clinical and operational efficiency as well as regulatory compliance of target programs. Clinical Pharmacy Specialists assume primary responsibility for leading departmental clinical education and research activities and serve as primary! preceptors for doctor of pharmacy students and residents.

JOB QUALIFICATIONS:
A. Education:
1. Graduate of an ACPE accredited College of Pharmacy.

2. Doctor of Pharmacy or other advanced degree highly preferred.

B. Experience and Training:
1. Registered pharmacist in the state of Illinois or eligible for same.

2. Completion of an ASHP accredited PGY-1 Pharmacy Residency and PGY-2 Specialty Pharmacy Residency required.

3. Four (4) or more years of clinical pharmacy experience in an organized health care setting preferred.

4. Exceptional aptitude or knowledge may be considered in lieu of experience. Extensive clinical experience and independent development in specialty area of pharmacy therapeutics may be considered in lieu of residency training.

5. Board Certification in area of expertise is required within 18 months of employment.

JOB RELATIONSHIPS:
Clinical Pharmacy Specialists reports directly to the Director of Pharmacy, unless otherwise designated.

Clinical Pharmacy Specialist has frequent personal and telephone contact with physicians, nurses, and other members of the health care team.

Clinical Pharmacy Specialist has! no direct reports but provides training and clinical mentorship of pharmacists and coordinates service line therapeutic programs.

JOB DUTIES:
Pharmaceutical Care:
1. Interpret, evaluate, and accurately process drug orders for patients receiving care in assigned geographical areas within the University of Chicago Medical Center.

2. Ensure the accuracy and appropriateness of all medication orders prior to dispensing of medications. Interpret the appropriate information required to identify each patient’s requirements relative to his or her specific needs and provide appropriate care. Contact prescriber as needed to adjust/modify medication orders and prevent adverse events.

3. Supervise the quality of relevant clinical programs and ensure all pharmaceutical services (clinical and operational) are well integrated throughout the overall medication use process.

4. Identify service requirements by interaction and consul! tation with patients, physicians, nurses, pharmacists, and technicians.! Provide information and education to healthcare providers regarding the safe use of medications.

5. Participate in the department’s chemotherapy quality assurance program which includes, but is not limited to, preparation of chemotherapeutic medications, monitoring of patients receiving chemotherapy, ensuring that all chemotherapy regimens are appropriate for individual patients based on disease, organ function, height and weight, body surface area calculations, age, etc. Exercise professional judgment in evaluating the safety and efficacy of all medication orders written for this patient population. Is the primary resource person for drug information for this patient population and is actively involved in educating patients regarding their drug therapy. Complete UMCM first and second check chemotherapy certification within one year of employment.

6. Clarify and solve problems concerning medication orders with prescribing physicians, suggest alternative! medication therapies when appropriate, serve as a source of drug information for health professionals.

7. Actively identify, mitigate, prevent, and report potential and actual drug-related problems or adverse drug events. Recommend rational therapeutic alternatives as needed to prevent future problems.

Clinical Practice:
1. Work collaboratively with other clinicians and multi-disciplinary team members to provide patient-centered care in a decentralized patient care model.

2. Provide clinical services for assigned patient populations and consultation as requested by the medical staff. Educate patients regarding safe and effective use of medications, possible side effects of their medications, and the management of these side effects.

3. Provide cost-effective pharmaceutical care by making recommendations that appropriately utilize time, medications, supplies and personnel.

4. Perform continual interventions on target p! rograms identified to improve patient safety, maximize therapeutic outc! omes, or control costs. Actively review and contribute to the redesign of intervention and target drug programs. Lead and design new initiatives and programs within assigned therapeutic service line.

5. Attend multi-disciplinary work rounds/care rounds with a designated service, if applicable to patient care. Establish new or expand existing multidisciplinary programs as needed or as requested.

6. Represent the Department of Pharmaceutical Services on relevant medical staff and hospital committees and teams. Attend meetings and other UCMC activities as defined or as needed. Serve as a liaison to these groups and report committee activities, actions, and resolutions back to the Department.

7. Investigate and document medication errors and adverse drug reactions as per guidelines defined by the Subcommittee on Pharmacy and Therapeutics. Prospectively and routinely review medication processes within assigned therapeutic service lines to maximize pati! ent safety. Implement action plans and system changes as needed or as requested.

8. Ensure proper drug utilization by adherence to drug use criteria and active participation in clinical pathways and quality assurance programs. Establish and monitor pathways and programs in assigned patient care areas. Contribute to ongoing drug utilization review activities and other quality assurance reporting programs by active support, participation, and leadership.

9. Document routinely and appropriately in patient care records (i.e. progress notes, verbal orders, etc.) as defined by policy. Communicate pharmaceutical recommendations to other health care professionals by direct verbal communication or by providing written documentation in the patient’s medical record when requested by the physician.

10. Ensure proper dosing of medications based on published dosing recommendations, general pharmacokinetic principles, and serum drug monitoring practice. Provid! e therapeutic monitoring of drug therapy including the monitoring of se! rum concentrations (i.e. Aminoglycosides, phenytoin, digoxin, warfarin, etc.) and the adjustment of drug dosages based on a patient’s age, size, renal/hepatic function, severity of illness, and other patient specific characteristics. Participate routinely with the pharmacokinetic dosing service and coverage of assigned patient care areas.

11. Maintain and enhance up-to-date professional knowledge of drug and drug therapy by reviewing relevant professional journals and suggested readings. Maintain expert knowledge of professional practice standards and regulatory guidelines from key healthcare stakeholders including but not limited to the Joint Commission, CMS, ASHP, ACCP, federal/state licensing/regulatory bodies, and UCMC and Pharmaceutical Services Policies and Procedures.

12. Document daily clinical and operation interventions as per the pharmaceutical Services Policy and Procedures. Emphasize interventions which prevent or eliminate drug-related prob! lems (i.e. medication errors, adverse drug reactions, drug interactions, etc.) or promote cost-effective drug therapy.

13. Actively participates in the Dr. Cart program. Participates in drug preparation and provides drug information as a member of the Dr. Cart Team during cardiac resuscitation.

14. Provide anticoagulation care to patients in a comprehensive acute care and outpatient practice. Work closely with physician, nurse and pharmacist colleagues to adjust anticoagulant medications, provide post-hospitalization and peri-procedural management, evaluate pharmacotherapy and provide patient teaching. Provide coverage of inpatient program for assigned patient care areas and staffing of outpatient clinic as requested.

15. Provide discharge medication review and counseling as appropriate (i.e. discharge medication reconciliation, home antibiotic teaching, etc.).

16. Understands and utilizes appropriate pharmaceutical care as it relates to ! the physical and psychological needs of neonatal, pediatric, adolescent! , and geriatric patients.

17. Maintain a high professional profile and represent the UCMC through professional affiliations and organizations. Maintain active membership and paid dues in the Illinois Council of Health-System Pharmacists, the American Society of Health-System Pharmacists, and a relevant specialty organization. Membership in other local, state, or national pharmacy organizations highly encouraged.

18. Maintain a high professional profile and represent the UCMC through regular publications, lectures, seminars and attendance at approved meetings or conferences.

Leadership:
1. Act as a patient care service line and therapeutics mentor, leader, and coordinator. Interact with Pharmacy Managers and Director to develop, refine, expand, and implement the UCMC model of pharmaceutical services and patient centered care.

2. Provides support to peers by assisting as needed, being a resource person and cross covering peers as! needed. Embody the spirit of teamwork within the department and act as an ambassador for the group approach to patient care

3. Contribute to creating a work climate that encourages positive staff morale, motivation and commitment for all who participate in the care of the patients. Participate in a changing environment by being supportive, cooperative and assistive with activities within Pharmacy and at the UCMC level.

4. Demonstrate reliability and accountability to Pharmaceutical Services, the UCMC and the Patient Services Sector.

5. Coordinate all clinical and operational work flow within assigned patient care areas.

6. Contribute to the practice of Pharmacy and the provision of patient care by offering constructive suggestions for change and participates in planning, implementing and evaluating new and existing programs.

7. Participate in Pharmacy and UCMC activities/goals to promote patient satisfaction and staff retention.!

8. Ensure hospital and medical staff standards are met or exc! eed for JCAHO activities related to medication management and Pharmacy and Therapeutics Committee. Lead key standard or performance metrics that assure compliance with JCAHO, ASHP practice standards/guidelines, federal/state licensing acts, and UCMC and Pharmaceutical Services Policies and Procedures. Document activities monitored by JCAHO/HCFA and other regulatory agencies as required.

9. Keep Department and Director of Pharmacy informed regarding changes in developing therapeutics or technologies as well as practice, policies, or regulations at UCMC. Identify and present to Department short-term and long-range obstacles to patient care and medication safety, compliance with regulatory standards, or stewardship of resources.

10. Act as secretary of assigned Subcommittee of Pharmacy and Therapeutics Committee. Responsible for coordination of Subcommittee agenda and minutes with primary committee via Director of Pharmacy.

11. Serve as an internal a! nd external liaison for the goals and vision of the Department of Pharmaceutical services. Embody a spirit of dedication and loyalty to the department and to colleagues.

Education/Research/CQI:
1. Develop and coordinate with the Pharmacy Managers & Director the orientation, education, and mentoring of new staff (i.e. pharmacists, nurses, pharmacy technicians, etc.) as needed. Act as a clinical practice resource, role model, and mentor for Department. Lead and assist with the development and improvement of the clinical pharmacy education program for all pharmacists.

2. Serve as a core faculty preceptor for the UCMC residency program and student experiential education program. Assist in the development and delivery of experiential training rotations for residents and doctor of pharmacy students. Assist with program development and establishment of relationships with local colleges of pharmacy. Provide timely and useful feedback to students and res! idents and communicate routinely with peers and Director regarding resi! dent and student programs.

3. Lead drug use evaluations and continuous quality improvement processes in the pharmacy and across UCMC annually. Maintain a list of issues to be examined. Responsible for analysis design, data collection and analysis, and implementation of changes. This might include working in both inpatient and outpatient areas.

4. Lead the review of therapeutics and medication formulary annually. Responsible for review of protocols, orders, and policies as requested. Develop drug monographs and other therapeutic resources required by the Department, Pharmacy and Therapeutics Committee, and medical staff. Act as a drug therapy and information resource for Department.

5. Design, lead, and provide in-service education on drug therapy to the medical staff, nursing staff and other allied professionals (i.e. dietitians, respiratory therapists, lab technicians). Include a review of therapeutics, policies and procedures, formulary, or regu! latory issues.

6. Design, lead, and deliver in-service education, journal clubs, and clinical pearls to pharmacists, pharmacy technicians, students, and residents. Includes review of therapeutics, policies and procedures, formulary, or regulatory issues.

7. Create and disseminate drug information bulletins for nurses, physicians, and staff.

8. Lead the creation, review, and revision of drug information resources and teaching documents for patients. Emphasis on medication counseling topics provided in an accessible and easy to understand format.

9. Participate in the preparation of articles for publication in the “Topics in Drug Therapy” or other UCMC newsletters as requested.

10. Provide consultation on all pertinent drug education either directly (via patient/family counseling) or indirectly (via critical pathway development).
